- A. We are conducting this recall to keep our customers safe and prevent injuries by removing the product.

- A. Look on the UPC label on the front of the package for item number 20-5749 (UPC number: 0 73525 57472 1).
- Q. How can I take advantage of the recall?
- If you are a consumer who has purchased this product from a store, contact Creative Expressions by phone (800) 428-5017 between 8 a.m. and 4:30 p.m. CT Monday through Friday. A customer service representative will send you a refund form in the mail, along with a pre-stamped, pre-addressed envelope to use for returning the form to Creative Expressions.

You will receive a refund for the retail purchase price of the product, along with your incurred postage cost within 4-6 weeks.
- Q. What is the problem with my product?
- A. Foil fringe can be removed from the bottom of the hat, causing a potential choking hazard for small children under three years of age. Since this item is intended for use at a child’s First Birthday Party, Creative Expressions has decided to voluntarily recall this product.
- Q. My product appears to be okay. Can I continue to use it?
- A. No. Even though your product appears to be safe, we are asking all customers to immediately stop using the product and participate in the recall by destroying the product and contacting us.
- Q. I no longer have the receipt for the product. Can I still receive a refund?
- A. Yes. You can receive a refund without a receipt by sending a completed refund form that includes the place of purchase.
- Q. Will I receive a refund for the product?
- A. Yes. You will receive a refund.
- Q. Can I return product to the store?
- A. No. Complete and print the refund form available on the Creative Expressions website, and Creative Expressions will process your refund.
